FormForge® - for Windows

Form Forge® is a PCL(Printer Control Language) typesetter developed by Alpine Data Inc to let you quickly and accurately create laser forms and macros for PCL-3 (HP LaserJet II or better) and/or PCL-5 (HP LJ IV or better) compatible laser printers and copiers like the Xerox Document Centers. Save the form as a standalone form or as a PCL Macro, both ready for text overlays. PCL laser forms result in very fast printing, much faster than with other formats such as bitmap, metafile or PDF, and the files are a fraction of the size. Form Forge takes advantage of the simplicity and portability of the PCL language to create laser form files with minimal effort. It's perfect for creating virtually any kind of form from Tax Forms and Invoices to Office Memos and Fax Sheets. Any form you can imagine can be built using Form Forge.
